Please bring with you a copy of recent blood test results
e.g. constipation, diarrhoea, nausea, fatigue, reflux, appetite changes, taste changes, etc.
Together with Julie you will identify dietary and lifestyle goals that are realistic and sustainable. You will negotiate strategies to help you to achieve these goals.
Educational resources and tools will be used to enhance your knowledge and understanding of food and nutrition in general, and as it relates to your medical condition.
Ongoing monitoring and evaluation is available to support and assess progress. Reviews also allow re-evaluation of goals necessary to address a change in medical condition and/or lifestyle.
Slowing disease progression and preventing chronic illness will let you spend money on the things that really matter, and give you a quality of life that allows you to enjoy them.
Some people may be eligible for medicare rebates if they have a chronic or complex condition and are referred by their GP under a Chronic Disease Management Plan. Essentially the plan allows for up to 5 Dietitian visits / calendar year – with a rebate amount of $52.95 per session (Item 10954). Full payment is still required at the time of consultation, and you will need to contact Medicare for your rebate. See your GP for more information or contact Medicare on 132 011.
DVA Gold Card holders will not have to pay for their visit to the Dietitian. However a referral from your GP is required. Please contact Julie to discuss the possibility of a home visit. Please note, you do not need a referral from your GP to see a Dietitian if you are a private patient.
Please contact Julie if you unable to make a scheduled appointment. At least 24 hours notice is required.
